Medical Treatment-Consent and Release Authorization: We the undersigned, for ourselves, our heirs, executors and administrators, waive, release, forgive and discharge Brock ISD, Brock ISD employees and coaches from all rights and claims for damages, injuries, or loss of person or property which may be sustained or incurred during participation in the Summer Strength and Conditioning Program activities while on Brock ISD property. I also give my permission for my child to be treated at a local hospital in case of emergency.
